Planetree International congratulates seven new Planetree Designated® Person-Centered Organizations. Designees embody the highest level of achievement in person-centered care defined by eleven core dimensions of a person-centered culture – ensuring every moment of the patient experience makes them feel cared for and cared about.
As one of 86 Designated® sites worldwide, these seven organizations contribute to the continued momentum and evidence-based support for person-centered care with a focus on a more holistic patient experience that includes family members, staff, and community. Planetree Designated® Person-Centered Organizations also drive quality improvement efforts in healthcare which foster better patience experiences by exceeding national averages in publicly reported Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) categories and outperforming national benchmarks on Centers for Medicare & Medicaid (CMS) process of care core measures.
Designated® organizations meet more than 60 criteria that reflect what patients, family members, and healthcare professionals say matters most to them during a healthcare experience. The following organizations represent the seven newest Designated® members to meet these criteria based on a rigorous validation process – including focus groups, data analysis, an on-site visit, and more – conducted by an independent committee of international healthcare experts and CEOs of previously designated organizations:
